Specification Form
- Please write the sample below on a separate unlined paper using a ballpoint pen only.
- State whether you are right handed, left handed, or ambidextrous.
- Write the sample in script (longhand) and if you usually print, provide both styles.
- Sign your full name below.
- For best results, make sure you are relaxed and seated comfortably.

We will call you and provide you with the personality profile on the phone. During the phone conversation you will have the opportunity to ask important questions focused on your specific needs. This recorded phone consultation will be mailed to you on an audio cassette tape or a CD within 5-7 business days.
